I am hoping someone can shed some light or even provide documentation regarding how to properly code Hypertensive Nephropathy. Some in my office state it would be coded as 401.9, 583.81. Some say that 403.90 captures it due to "Includes" note listed under 403 in the tabular and the Index leads you to 403. Please, any help would be greatly appreciated. I would just like to know the correct way to code it.
 
Hypertensive neuropathy should be coded 403.XX. If associated CKD, it must be coded as well.
