I am not sure how to code "tandem lesion of 80% - 90% involving mid right coronary artery" from a cath report.

Typically we use I25.118. Another coder had suggested using I25.83(coronary atherosclerosis due to lipid rich plaque) or I25.84 (coronary atherosclerosis due to calcified coronary lesion); however, physician said that he cannot be sure if blockage is due to lipids or calcification.

Any suggestions?
 
I would code I25.10 CAD of the native artery without angina, UNLESS angina is documented in the report. If it is then I would assign the correct cad with angina based on the type of angina.

I would NEVER assign i25.84 or i25.83 with physician documentation in the report.
