A patient presents for a lap cholecystectomy as a ambulatory surgery patient. They are brought into the OR, trocars are placed in the abdomen to begin the lap cholecystectomy. The procedure is cancelled as the surgeon is called away for an emergency. Then a few hours later the same patient is brought back to the OR and a lap cholecystectomy is performed?

Should I code only a lap cholecystectomy for this entire scenario? Should I query the surgeon to find out if they have coded and billed for two lap cholecystectomies with one having a 74 modifier? Would I consider using a 76 modifier for the second completed lap chole?

This is a stumper for me.

Any help is greatly appreciated.

Yikes! There's isn't going to be a perfect coding scenario that is going to make this easy. More than likely, you are gonna have to appeal.

First time around you could try:
4760*
47600-52-59

Second:
4760*
47600-52-59-76

Good Luck!
