Pt. had a surgery a year ago by someone else
Diagnosis: paraspinal abscess
Procedure: Debridgement of paraspinal abscess lumbar spine down to bone
Physician opened the previous incision. A large subfascial collection of purulent material was aspirated. The walls of the cavity were debrided all the way to the lamina and facet joints.

I'd code it as an 11044 - of course depending on the closurer, you can code that as well (intermediate or complex/depending) with a modifier of course! that's what I'd do anyway...
