Good Morning,

Can I take the HPI from the Chief Compliant if the physician does not document it? Or do I bump a 99213 down to 99211? Does any have any info from CMS regarding this? I am located in NYC.
 
Good Morning,
The best approach is to talk to the provider before you code the visit. there may be a technical issue or they may have overlooked to document.
Never assume that the claim is ready to be coded without the research and communication, you could loose out on revenue and being compliant.
