Wiki HPI, Chronic, and the 1995 Guidelines

Sometime it is difficult to dissect out the chronic HPI or chronic status notes from the rest of the HPI note.

So can a chronic disease or illness to count, or to be counted, under the 1995 guidelines?

Do we have to leave out everything in the HPI related to a chronic illness with the 1995 guidelines?

Help? Feedback. :confused:
 
The medical record should describe elements of the present illness (HPI) or associated morbilities (1995 guidlines)

The medical record should describe elements of the present illness (HPI) or the status of atleast 3 chronic or inactive consitions (1997 guidlines)

Thank you, those are good to remember. It is just hard to separate chronic from present sometimes. I think it is why lean towards 1997 Guidelines.
