HISTORY OF PRESENT ILLNESS:LOCATION:Breast , QUALITY:Discomfort, SEVERITY:Mild,DURATION:Longtime

Physician Notes :breast pain and discomfort.

Please suggest me How many HPI elements we can take from the above documentation.

I think that the CC should be discomfort so the HPI elements would be location and duration
