Wiki HPI elements - complains of sore throat

Would you count the following statement as severity for the HPI? "Pt complains of sore throat, severity is 0 out of 10." I haven't been giving credit for this statement, but maybe I'm overthinking this.

Thanks! I would absolutely give credit for 5 out of 10, but it kinda seems like they weren't really answering the question. I guess I really am overthinking this.
 
I would talk to the provider

That is conflicting information, how can you complain of pain with no pain present?

Likely just a typo in the severity portion but I would bring this to the providers attention. Especially since it sounds like this has happened more than once.

That's kinda what I was thinking. I see this occasionally, and I'm not sure exactly what they mean. I will discuss this with our Medical director.
