What elements of HPI would you get from this senario?:

Patient presents for recheck of diabetes. Patient has been diabetic since she was 12. Patient monitors her blood pressure daily.

Recheck of diabetes is your CHIEF COMPLAINT
Diabetic since age 12 is DURATION
Monitors B/P dailiy is ASSOCIATED SIGN


Now if the physician had said Patient has Type II diabetes you'd also have QUALITY.
If the physician noted the range of B/P or blood sugars per patient's home records you'd have (e.g. BP has never been higher than 120/70).
