I have a CC & HPI that states:

CC: patient presents for recall colo.

HPI: No nausea, vomiting, or constipation. Moves her bowel 3 to 10 times daily but not diarrhea for the last year or so. No black or bloody stools. Appetite is good with weight stable. No abdominal pain, heartburn, or swallowing issues.

I am able to give credit for: Location, duration, and associated signs and sx. Am i missing any other elements?
 
Timing-Okay. What about "appetite is good with weight stable", could this be severity? And "black or bloody stools" be considered quality? I always sencond quess myself in the HIP section.
 
I don't see a complaint to pull any HPI from

These are just ROS in my opinion, there is no chief complaint that indicates a problem.
