I'm wondering if anyone has insight to share,please
Patient was scheduled for a PAP with NP and her AW was done prior by her PCP
Looking at the HPI the NP did document an expanded focus history with abn pap> than 10 yrs ago post cone bx and D&C all other paps wnl, ex smoker,sexual activity hx,mother deceased breast cancer,previous GYN exam were done with Gynecology
Vitals obtained,general appearance
the exam was 9 bullets for pelvic exam
breast exam was documented with 5 bullets
Plan
pap obtained and sent
I used the G0101 and Q0091
I'd like to use 99212 E&M as well
Also other than z12.4 is there any other ICD code that would be appropriate
