can the hpi come from anywhere with in the documentation?
 
Yes IF

Yes, IF it was recorded by the provider. (Ancillary staff or patient cannot record the HPI.)

Some providers just dictate one big paragraph (SIGH...) and the coder is left to figure out which phrase is counted for which part of the documentation. Just be careful of double dipping!
