Can someone tell me where on the Medicare website you can find that the HPI must be documented by the physician and not ancillary staff?
 
If only those magic guidelines were available! Actually, what the documentation guidelines state, are that the ROS and PFSH may be documented by ancillary staff (note the absence of the HPI).
