we use V04.89 & yes...so far it's covered by insurances. (sometimes free so append the SL mod) depending if state supplied

V04.89 and yes we are getting paid. There is an age limit however. I think it's 12-26yrs of age but I can't recall. If you check the CDC website it has it.
