It is documented this pt has ckd due to hypovolemia during surgery, and also has hypertension. Guidelines state to assign hypertensive chronic kidney disease when conditions from 585 are present with hypertension, but the provider has clearly stated the CKD is because of blood loss, not hypertension. It just feels odd to code the one without the other and just want to see some kind of consensus. Any references gladly taken.
 
ICD-9 presumes a relationship when a patient has both hypertension and chronic kidney disease. There's an exclusion directing you not to assign these codes if the condition is not due to hypertension. In this case, you are right not to code hypertensive chronic kidney disease since the provider specifically states there is no cause-and-effect relationship.

The use of codes from categories 403 and 404 does not apply when:

-The renal condition is acute kidney failure
-The hypertension is described as secondary
-The kidney disease is specifically stated as due to a cause other than hypertension
