Question:

When a patient presents with HTN/CKD4/DM, the guidlines for Hypertension assume the cause and effect link between the HTN and CKD, (403.90, 585.6) is there any assumption or need to use two combination codes for the CKD and DM ? diabetes combination code ? 250.40 or just 250.00.

Any thoughts or suggestions ?

If the physician documents any cause and effect relation between CKD and DM, we can go with 250.40, There is no guideline to assume the relation of these two conditions DM and CKD. Its better to code 403.90, 585.4 and 250.00 for the above scenario.
