Humana continues to deny Factor IX for patients that are not hemophiliacs. I have a patient on Eliquis for afib who came into ER with subdural hematoma. Patient was given Factor IX as anti-coagulant. Humana advised Factor IX only approved for hemophiliacs. Does anyone have any literature, peer reviews, policy bulletins, etc to support an appeal? Thanks.
