Is there any other provider offices that are part of the HCC Bundling program for Humana? I am trying to obtain information in regards to if you have to actively be treating a patient for a dx code in order to append the dx code to an office visit. Humana is advising that the HCC needs added per this program even if we are not treating the patient for the diagnosis. We are only reporting the active diagnosis. Any information that anyone can provide regarding the Humana HCC Bundling program would be greatly appreciated as our office is receiving conflicting information in regards to HCC's.
