Humana - I am having trouble

I am having trouble with humana paying dme

l3485, l1930 with an lt modifier, they are denying saying it is
an inappropriate modifier. Any recommendations,
should i be using an nu?

Also denying claims when i use a pos 32
denying with incomplete info regarding place of
treatment.

I have never had this problem with any other insurance.

Modifier KX may be the answer if it meets the requirements. If not, then modifier GY.
