We have billed out 80061,83721 for years. Recently Humana Medicare has stopped paying us for 83721, stating it is included in 80061. 83721 is billed with a 59 modifier, as we have always billed it. We have a CLIA certified, in house lab, so only some are sent out. These are done in house. Can someone help?
