I am having trouble with Humana paying 80101-QW. The reason they give is: This amount reflects a contracted agreement with the provider. Claim specific negotiated discount. Any help would be appreciated.
 
I had to start billing all BCBS plans with a G code. G0431-59 x 1 unit (if CLIA waived: G0434-QW x 1 unit) Included in that are all of the Medicare fee for service/replacement plans. Now the BCBS insurances are paying.
