If a dx says History of Asthma stable and History of GERD stable. Does any one codes these as current condition. (No code for hx of.)

The patient came in with fever and ear pain and the primary dx was URI.If the other problems are stable do you have to code them.

Any thoughts would be helpful.

If I'm reading your question correctly, this is what I would do: Assuming this is the same patient/same visit and the chief complaint is ear pain and fever but is diagnosed as URI which is not effecting or effected by the (hx of) asthma or (hx of)GERD, 465.9 would be the only appropriate code. :)
