I have a resident who has a history of a hip fracture but no documentation whether its pathological or traumatic. If it helps, this person also has osteoporosis...

Which code do I use if the documentation in an unspecified... but there's no unspecified code for the history of a hip fracture?

if it does not specify then you cannot code for the hx of fx. you can code the osteo.
