I'm in Michigan with having a difficult time getting my claims with CMS (Medicare) paid using Z08 or Z09 like we were instructed. I have billed multiple claims example Z09(1) Z86.0101(2) with G0105 as the patient has a history of polyps or Z08(1) Z85.038(2) with G0105 patient has history of Colon Cancer. Is there something I'm doing incorrectly? Any help would be greatly appreciated. Thanks
 
Hi, I'm in Cali.............. I did a quick search out your way, looks like your LCDs are asking for a different range of dx..................... Try leading with Z12.11 in place of Z08/Z09.

I see this on your list for HIGH RISK...........
