I see that Novitas LCDS state that less than 500 ml of normal saline is not considered reasonable and necessary.
Does this include pediatrics?

Medicare guidelines are for adults. Pediatric hydration is typically given at 20 ml/kg so if 250ml were administered, the child likely weighs about 27 pounds. If the patient was dehydrated and required IV hydration in lieu of oral rehydration, then codes 96360/96361 would be appropriate based on the time of the service. I hope that helps. Cindy
