do you code hyper if symbol up arrow is used and hypo with a down arrow?
 
NO - not at all

Just because a patient's blood pressure (or glucose level or cholesterol level) is higher or lower than last time does not give him/her a diagnosis.

The PHYSICIAN must state the diagnosis specifically for it to be coded.
