One of my doctors documents 1) Hyperglycemia, DM Type II, Controlled...... isn't that contradictory?

It is. At the same time, I am thinking that pt has a known condition of DM with hyperglycemia and the provider is meaning that conditiin is under control., he is not ready to say just DM as of yet. It depends on A1C value and provider judgment. Query him if possible.
