Wiki Hypertension with CKD and or heart disease

Hello everyone,

I am hoping to gain some insight on coding for Hypertension with kidney/heart disease. I understand the guidelines pretty well I think, so I think it's more of an issue with the documentation or with the way our notes are printed. We are still on paper charts and our progress notes have preprinted diagnoses that the doctor can check off, one being HTN and CKD without heart failure or with heart failure. Would this code as I13.10 or I13.0 depending on whether it's with or without heart failure? The reason why I am confused I think is because it doesn't specifically state that the patient has hypertensive heart, it just says Hypertension and if that's the case, wouldn't it code I12.9 if there isn't heart failure but is CKD? I hope this makes sense and i can try to clarify if needed. I attached a snippet from the progress note hoping a visual would be helpful. Please help!
