I think I already know the answer to this but I am going to ask for reassurance... We had a patient come in for a blood pressure check and the doctor wrote an up arrow then BP. The patient has previously been diagnosed with Hypertension but since every note is supposed to stand alone, I wouldn't be able to code it this way because he didn't specifically document Hypertension, correct? I do want to add that the provider did add HCTZ to his medication regime of Lisinopril. Based on that info, would that make a difference? Thanks!!
