If documented correctly can you code 404.91 hypertensive heart and CKD with heart failure and with CKD with code 402.01 malignant hypertensive heart disease with heart disease -

no you do not code these together, if it is malignant hypertensive heart and chronic renal failure then you only code the 404.01 you will need the 585.x code for the chronic kidney disease as well as the 438.x code for the heart failure.
