Can anyone please offer guidance around this ?
Example: If you found ‘hysterectomy’ documented in a medical record and did not know if the cervix was removed, how would you code that? Is there more than one code for this situation? (CPT, ICD10, SNOMED CT)

What exactly does the documentation state? If it states a total hysterectomy, then the cervix was also removed. What exactly do you mean by "found 'hysterectomy' documented in a medical record? Like as a history of hysterectomy?
