Wiki I have an issue trying to get Humana to pay multiple fat grafting units

I have used the mod-GD to bypass the MUE and the procedures are clearly documented in the notes in my opinion. I need some suggestions for a different approach with Humana.

I have attached the notes for review.

Any help would be greatly appreciated. Honestly, I think his note is inadequate, but am not sure how he can improve his documentation.

I have only billed the 20926 code once for each breasts, so if both breasts are done at the same time, then I submit code 20926 with qty 2. It is the understanding from my plastic surgeon and I that no matter how many times the fat is injected into the breast, it can only billed once.
