Hello,
Getting denials for MCR HMO when I use an I87.3xx dx with a I83.xxx dx (have both conditions. Has anybody else in Florida had this occur? Why would carrier bundle Hypertension w/ulcer be bundled together with postThrombotic w/ulcer or varicose vein w/ulcer. Do not see any guidelines, any ideas? thx
