We have some codes that are for "multiple sites" but not "multiple UNSPECIFIED sites"

For example, arthropathy....

Suppose a patient has arthropathy in the left elbow and the right knee...

Do you code three codes, one for left elbow, one for right knee, and one for multiple?
Do you just code the Multiple?
Do you just code the specified sites, and not code the Multiple?

My point is...if they want the "multiple sites" code to be used only when the sites are not specified, they should have worded it "Multiple UNSPECIFIED Sites"

If they want it to be used ONLY in addition to specified sites coding, then it should have been worded "Multiple SPECIFIED Sites"

And if the site is unspecified...would you code for "Unspecified" as well as "multiple"?

This wording is a bit of a problem.
 
Wouldn't the guidelines about coding to the highest level of specificity inform us to code only the specified sites, in this case?
