What would you dx code for a pathology report with final dx of a renal biopsy with ARTERIAL AND ARTERIOLAR NEPHROSCLEROSIS. I've been coding I12.9 and N18.X. Not sure if I'm right or wrong?

if you look under the I12 includes note has nephrosclerosis and of course per guidelines if using this code the
CKD must be coded
