I was wondering if anyone has any expertise on how to code a colon polyp (either tubular adenoma or hyperplastic polyp) with inflammation. This is not an "inflammatory polyp". The exact diagnosis was "eroded and mildly inflamed hyperplastic polyp". Should it be coded as K63.3 for the erosion, K52.9 for the inflammation, and K63.5 for the hyperplastic polyp...or just one of these? Thanks in advance.
