I apologize if this question is in the incorrect forum section. I am new to HEDIS measures reporting. Would someone please help me understand how diagnosis data is captured from HCFA 1500 claims for HEDIS reporting purposes? Are insurance payors capturing the ICD-10 data reported in the diagnosis header fields 21A -21L on the HCFA form or only from the diagnosis pointer field 24E on each service line billed?
