When a patient comes in for renewal on medication for htn,ddm and hyperlipidimia no changes what would you code first ? Little confused on icd 10 i work for a primary care solo dr. Any help is very appreciated. Thank you in advance.
 
There is no guidance within ICD-10 that shows sequencing of codes for this type of example. I go with the most significant primary and then work my way down to my Z codes.
Example:
Exacerbation of COPD
End Stage Renal Disease
Hypertension
Dependence on dialysis machine
