Wiki ICD-10 Physician Order regulations

I work with Long Term Care facilities and am wondering what the physician order regulations are for 10/1 conversion. Currently the facilities add the ICD-9 codes to the physician orders which then gets sent to the pharmacy. New orders effective 10/1 and after will have the ICD-10 codes.

What does the facility have to do with the existing orders that have ICD-9 codes on them. Do they all have to be changed to ICD-10? Do they have d/c the orders and then enter new ones with ICD-10 codes?
