Hi,
I'm confused as to the combination codes in ICD 10 BOOK of HTN and CKD as noted in Chapter 9: Diseases of the Circulatory System (100-199)

a.) Hypertension 'these conditions should be coded as related even in the absence of provider documentation explicitly linking them, unless the documentation clear

Versus notes:

2) Hypertensive Chronic Kidney Disease ' CKD should not be coded as hypertensive if the physician has specifically documented a different cause'.

I agree those passages could have been written better, but they are consistent

In other words, ALWAYS link hypertension to chronic kidney disease UNLESS the provider specifically says the CKD is from another stated cause

99.9% of people with hypertension and CKD will have the combination code used
