Hello,

I'm a compliance auditor and upon reviewing an encounter I see the provider reported ICD-9 250.90 Diabetes with unspecified complication. In the note their is no correlation of a manifestation due to the diabetes. Im Confused :confused:

Can someone share examples or where can I find guidelines on when to use this DM code appropriately.

The other DX reported were 401.1, 272.2, 729.82

Any help will be greatly appreciated ;)
 
Read your coding guidelines, the provider must document a complication in order to code a diabetes with complication code.
Any dx code you submit must be documented in the medical record note, not just selected as a code by the provider.
