Surgeon replaced bi-V ICD generator using existing leads. Then he removed faulty LV lead and was not able to secure a new LV lead. He opted for new RV septal lead to be placed in the left bundle position. Do I bill 33249, 33225-53, 33241, 33244 or 33264, 33225-53?
