we have a patient coming for dual chamber icd gen change cpt 33263 but its not at eri. patient needs to get chemo so the physician is going to change pocket from left side to right side but also wants to do gen change. I checked cms site and it does not give me any information about this type of coverage. has anyone had such scenario in past? patient has medicare insurance
