can you bill this code 764.00 for a 10 year old child if the physician wants to show history of SGA?
ICD9 guidelines seem to say yes but my coding software and payers say no...not age appropriate. What gives?
 
764.00 is a newborn code and can be used up to 28 days old. It is not appropriate for a 10 year old. What is the diagnosis you are trying to code? there is no code for history of SGA.
 
The Medicare Code Editor icons that show in some coding references should not be confused as ICD-9-CM guidelines and should not be applied to coding of professional services. Chapter 15 provides codes for conditions that originate in the perinatal period not just for conditions occurring in the perinatal period.

Codes for perinatal conditions may be used throughout the life of the patient if the physician documents that the patient is currently affected by that condition. See Coding Clinic of 1st quarter 2009 for a similar question regarding use of codes for prematurity in for a 2-year-old patient. The answer to the question was, "Assign code 765.23, 25-26 completed weeks of gestation, as an additional diagnosis to indicate that the child was born at 26 weeks. Codes from Chapter 15 should be assigned when the provider has indicated that prematurity is a contributing condition, even though the baby is not still premature at the time of the current encounter."

History of perinatal problems is reported with code V13.7. If a physician wishes to indicate the patient is affected by low birth weight, codes V21.30-V21.35 are appropriate.

If the Physician only references that the now 9 month old was 27 weeks Premature and has PVF and MADD but makes no reference to birth weight,does that mean do not code for the birth weight ( discharge notes show 950 grams). Do we only code 277.85, 779.7 and 765.24 or could we code the 765.03 or V21.32. If he had referenced the low birth weight how do you determine to use code from 765.0X or V21.3X? Thanks!
