Pt goes to surgery every 3 days for recurrent infection in wrist and hand. Previous sutures removed, exploration, irrigation and nonviable tissue removed. I have coded 25028 and 26025 (hypothenar irrigated). The codes don't look right because there is no incision, only removal of previous sutures. I am not able to find better codes. Please give suggestion/advise if codes are correct in this case or any better codes. Thanks in advance.
 
I had a complicated case like this once with multiple fingers, wrist, etc. I believe I used a lot of debridement codes (like 11043 or 11044) over and over, and I believe there were some arthrotomy codes for the joint explorations, and possibly some tendon sheath incisions, and lots of different dx codes!
