Thrombectomy for dialysis access - left leg

The operative site was prepped and draped in the usual sterile fashion. Local anesthesia was injected. Fluroscopy was used as needed during the procedure. A small incision was made over the access and it was dissected out. The access was opened and the clot was removed with a #4 embolectomy catheter. The arterial plug was removed. The access was flushed with heparanized saline and clamped with vascular clamps. The opening in the access was closed with 6-0 prolene. clamps were removed. Hemostasis was achieved. The wound was closed in 2 layers with absorbable suture. Flow was now present in the access.

Would you consider this an open thrombectomy - any suggestions appreciated.
 
Is this for a arteriovenous fistula? If so then 36831 would be correct. Flouro for AV dialysis graft is 77001-26. But if this for guidance then it is bundled.
