Under 2021 E/M changes if patient comes in and sees a nurse to receive an immunization only can or should we be adding an E/M code with it? Our system now defaults to 2021 logic/methodology and it is automatically adding 99212.

I would say no, there should not be an E/M billed. You're being paid for the nurse's time giving the injection in the immunization administration fee. If there is a separately identifiable service supported by the medical record, that's a different matter.

And the system "logic" that is allowing a level 2 visit to be generated for this is concerning to say the least.
