Hi! looking for a little guidance. What if a patient is coming in to have A flu shot, urinalysis, annual exam, and a problem is found on exam that causes further follow up? The 99212(3) would have a separate dx code based on the issue.
For example:
81002
90686
90471
99395
99212(3)

This was attempted with a 25 mod on both E/M codes, due to the immunization and using them together, but the 99213 was denied due to benefit being included in another service. how are we able to bill all of those together?

Any help is appreciated!
