I'm getting many KS Medicaid denials for immunizations, in particular, use of 90658. KMAP explanation is code 1307. I'm looking for advice on what we're doing wrong.
 
There are new NCCI edits that bundle this into E/Ms. Did you bill an E/M, including preventive services, with the immunization? If so you will need to append a 25 modifier to the E/M. These are suppose to be corrected, but until use the 25 modifier.
 
Sorry - newbie here. With the modifier on the E/M, do I also use 90658? What should be done when someone only comes in for a flu shot?
