90723 has 5 components so you have 1 90460 and 4 90461; 90645 and 90680 each have one component so you have 2 more for 90460:
90460 x3
90461 x4
You should have 90460 the same amount of times as the actual immunizations given; 90461 will be the number of additional components in each immunization combo.
